Findings: The study reveals:
-Lumbar spinal curve: Normal lumbosacral vertebra spinal curve.
-Spinal column: Normal signal intensities in T1WSE,T2WSE of almost all levels of lumber spines with normal uniformly signal T1WSE, T2WSE of bone marrow vertebral body.No eevidence of bone marrow infiltrative disorder or bony destruction. No subluxation, translation or spondylolisthesis of lumbar vertebrae.
-Intervertebral disc: Normal intervertebral disc height. Decrease SI T2W of nucleus pulposus of L3-4 and L4-5 levels are noted which are suggestive of degenerative discs.
-Extrusion of L4-5 disc in central and left paracentral aspect are noted.
-High SI T2W in posterior aspect of anulus fibrosus of L3-4 could suggestive of anular tear.
No evidence of spinal cord compression.
No evidence of bony destruction or paravertebral mass.
-Spinal cord: The study of spinalcord in conus medullaris shows normal size and signal intensity. No extradural cord compression. No intradural and intramedullary lesion. Normal signal intensity of the spinal cord parenchyma.
-Axial L1-2: Normal disc. Normal spinal canal. Normal bilateral lateral recess and neural foramen.
-Axial L2-3: Bulging disc. Normal spinal canal. Normal bilateral lateral recess and neural foramen.
-Axial L3-4: Bulging disc with anular tear without nerve roots compression. Normal spinal canal. Normal bilarteral lateral recess and neural foramen.
-Axial L4-5: Extrusion of HNP L4-5 centrally and left paracentral with mild compression to spinal canal. Left lateral recess compression (left L5 root)could not be excluded. Normal right lateral recess and neural foramen.
-Axial L5-S1: No evidence of HNP. Normal spinal canal. Normal bilateral neural foramen.
MR Myelography: Extradural compression at left L5 root.
Impression: HNP L4-5 in central and left paracentral location with suspected of Left L5 root compression. No intramedullary lesion.
